PLEASE ARRIVE NO EARLIER THAN 15 MINUTES PRIOR TO YOUR APPOINTMENT.  

CLICK the website link in order to select an open time slot, and print the consent form to bring to the vaccine clinic. Be sure to have it filled out with a copy of your ID and current insurance card.  If you do not have insurance, we will still provide your vaccine.
